【2023.10.26新增】治癒14歲、有貓愛滋病毒(FIV)帶原的口炎母黑貓

這是一個很棘手的貓慢性齒齦口炎 (Feline chronic gingivostomatitis, FCGS,以下簡稱貓口炎) 案例,在我們完美治癒的所有貓口炎病患中,創下「治療最久」和「住最遠」兩項紀錄。

麥麥是一隻已節育的混種母黑貓,飼主住在
屏東恆春,患有嚴重口炎 (FCGS) 的同時,也是貓愛滋病毒 (FIV) 的帶原者,口炎導致麥麥的食慾明顯減退、活力不佳;在先前的醫院使用類固醇控制病情已有兩年,卻已經開始出現藥物耐受性,飼主遂帶貓來本院接受陳重威獸醫師獨創的牙齦黏膜置換手術 (Mucogingival Replacement Surgery, MGRS) 進行口炎 (FCGS)治療,開始治療時貓咪是12歲,牙齦黏膜置換手術 (MGRS) 療程結束時14歲3個月。

由於患貓本身是貓愛滋病毒 (FIV) 帶原者,會影響到手術後的癒合狀況,整個療程中,陳醫師數次調整麥麥的療程,包括手術方式、維生素D的劑量等,嘗試促進她的口腔癒合速度。另外,也請飼主限制麥麥的活動區域,不讓她再自行跑出去家門外,避免接觸到外界不明的致病因子,影響治療的成效。


經過2年3個月的牙齦黏膜置換手術 (MGRS) 治療後,麥麥的口炎 (FCGS) 終於完全治癒,食慾良好,精神活力極佳,正常生活即可,不需要再使用藥物控制症狀。

由於麥麥本身是貓愛滋病毒 (FIV) 帶原者、也患有嚴重牙周病,雖然口炎 (FCGS) 順利治癒了,但還是需要每天刷牙及定期洗牙,保持良好的口腔衛生,才能確保口腔的健康。

整個療程非常的漫長艱辛,很感謝飼主願意相信我們並堅持到底,最終順利完成療程。

貓咪慢性齒齦口炎(FCGS)是常見的貓咪口腔疾病之一,常見的症狀為:
1. 牙齦及口腔黏膜紅腫,甚至潰瘍或增生,口腔所有軟組織,包含牙齦、兩頰舌頭兩側、舌頭下方、嘴角至咽喉部的口腔黏膜都會有紅腫潰瘍或增生。
2. 貓咪會因為口腔疼痛會減少進食,甚至不願意進食,或是吃飯喝水會不斷甩頭、使用前腳撥動嘴巴。
3. 口水因潰瘍多半都有異味,甚至帶有腐肉味。

目前普遍的治療多為施打長效型類固醇針劑或是口服類固醇藥劑,搭配抗生素,以及拔牙,但個體治療效果落差較大,其中類固醇容易造成多種副作用與藥物耐受性,而拔牙容易使貓咪口腔塌陷、齒槽骨萎縮,根據研究統計,高達78.9%的貓咪,拔牙後仍需要繼續打針吃藥控制症狀,即便是拔牙後症狀有改善的貓咪,也還是有68.8%的比例需要繼續打針吃藥。想知道更多資訊可參考這篇文章:https://reurl.cc/v6E6ON

【Added on October 26, 2023】: Healing 14-Year-Old Black Female Cat with Feline Immunodeficiency Virus (FIV) Carrying Feline Chronic Gingivostomatitis (FCGS)

This is a tricky case of Feline Chronic Gingivostomatitis (FCGS), hereinafter referred to as stomatitis, setting two records among all the FCGS cases we’ve perfectly treated: “longest treatment duration” and “furthest distance from our hospital”.

Mai-Mai, a spayed mixed-breed black female cat, lived with her owner in Hengchun, Pingtung. She suffered from severe FCGS and was also a carrier of Feline Immunodeficiency Virus (FIV). FCGS had significantly reduced Mai-Mai’s appetite and vitality. Prior to coming to our hospital, she had been on steroids for two years in another animal hospital, but she had developed drug tolerance. Consequently, her owner brought her to our hospital for Dr. Chung-Wei Chen’s innovative Mucogingival Replacement Surgery (MGRS) for FCGS treatment. At the beginning of her treatment, Mai-Mai was 12 years old, and at the end of the MGRS treatment, she was 14 years and 3 months old.

Since the cat was an FIV carrier, it affected the healing process post-surgery. Throughout the treatment, Dr. Chen adjusted Mai-Mai’s treatment plan multiple times, including the surgical approach and vitamin D dosage, to expedite her oral healing. Additionally, the owner was advised to limit Mai-Mai’s outdoor activities to prevent exposure to unknown pathogens that could affect the treatment’s effectiveness.

After 2 years and 3 months of MGRS treatment for stomatitis (FCGS), Mai-Mai finally made a full recovery. Her appetite returned, her energy was high, and she could lead a normal life without the need for symptom-controlling medication.

Despite the successful resolution of FCGS, Mai-Mai, being an FIV carrier with severe periodontal disease, still necessary to brush the teeth daily and have regular dental scaling to maintain good oral hygiene and ensure oral health.

The entire course was exceptionally long and challenging, and we’re grateful to the owner for trusting us and persevering until the end, successfully completing the treatment.

Feline chronic gingivostomatitis (FCGS) is one of the common oral diseases in cats. The main symptoms are:
1.     The gingiva and oral mucosa are red, swollen, ulcers or hyperplasia. All soft tissues in the mouth, including the gingiva and all parts of the oral mucosa (including buccal, lingual, sublingual and oropharynx, etc.), will have redness, ulceration or hyperplasia.
2.     Cats will lose their appetite or even refuse to eat because of mouth pain, or they will keep shaking their heads and scratching their mouths with their front feet when eating and drinking.
3.     The patient's saliva usually has a peculiar smell, even putrefactive odor, due to ulcers.

At present, the most common treatments are long-acting steroid injections or oral steroids, combined with antibiotics, and tooth extraction, etc. However, individual responses to these treatments vary, and steroids can lead to multiple side effects and drug tolerance. Tooth extraction can result in oral collapse and alveolar bone loss. Studies show that up to 78.9% of cats still need injections or medication even after tooth extraction, and even in cases with symptom improvement, 68.8% still require ongoing medication for treatment. Dr. Chung-Wei Chen's original Mucogingival Replacement Surgery (MGRS) aims to solve the above-mentioned sequelae and difficulties in the treatment of stomatitis, and make cats with FCGS no longer need to take medicine for a long time after recovery, and return to the normal life they had before the illness.
